This is a screenshot of the new myVATSIM portal, on the ICAO flight plan form page. Previously, an old FAA one was used as VATSIM was a mainly American network, but as it has grown to become more global, the FAA flight planning format has been deprecated recently in favour of the ICAO flight planning format, including in North America.
